Case Management

Case Managers stand alongside individuals in need and work on their behalf in a way that represents the best interest of that person.

The first phase of the program involves locating and building relationships with potential clients. The purpose is to provide them with immediate support, intervention, training, and connections with community agencies and/or mainstream social services, housing programs, nonprofits, and local churches and ministries. From there, we make an initial assessment of their needs and eligibility, provide crisis and housing counseling, and address urgent physical, emotional, and spiritual needs. Next, our case managers attempt to maintain relationships with clients that are essential to help the individual or family stabilize. They also work with the client to confirm chosen services are appropriate, add or change services, negotiate to resolve existing problems, and provide relevant information and advice concerning services received. The end goal is to keep families together, help clients maintain or establish permanent housing, and sustain employment. All of which will ultimately strengthen our communities.

With the goal of self-sufficiency in mind, we require each unemployed client to attend classes in order to receive financial aid or support services for such pressing needs as rent, security deposits, and utility shutoffs. We also assist with clothing, baby supplies, medication, IDs, birth certificates, and other basic needs. Case managers register clients for our Earn While You Learn (EWYL) classes during intake. These classes teach critical life management skills that may never have been learned or have been lost during the course of physical or mental illness, domestic violence, substance use, incarceration, and homelessness. They are essential for program participants to function independently in the community. This is very similar to the Department of Social Service’s Temporary Cash Assistance Program—also known as TCA. In order to receive TCA, clients must volunteer approximately 20-30 hours per week for the duration of their enrollment in the program.

Mandatory classes include Money Management and Parenting, otherwise, clients are free to choose the classes they want to attend. They cover a range of topics such as how to set healthy boundaries, budgeting, retraining your brain to get rid of toxic thinking, and using art as a method to set goals and think outside the box. We also have classes that help clients find out about their unique personalities, talents, and abilities and how being in their optimum environments can help them be successful in life.

Our center also offers a computer lab for our clients’ use. The lab creates an environment for self-paced learning. It is also a tool for quick and easy access to secure sustainable employment and professional development. Our Draped with Confidence boutique compliments our efforts by empowering men and women in their determination to transition from training programs, unemployment and/or under-employment to self-sufficiency by providing free professional clothing and training workshops.

What to Expect: 

Starts with an initial intake on client to gather information which lasts approximately 1 hour. Client will take up to four self-assessments in order to gain additional insight into their situations and how the case manager can target areas to improve their self-sufficiency.   4. Transportation Assessment

After assessments are completed, the case manager and client will review the TVC FSS Evaluation Matrix and begin their first rating. Afterward, they will decide together which three benchmarks they will begin working on to help the client reach self-sufficiency.

The matrix is a proven case management tool for case workers, instructors, and mentors as they work with individual clients and document progress towards self-sufficiency over time at specific intervention points or, when appropriate, to document a client’s ability to maintain a certain level of functioning.  It also helps document progress or maintenance of client skills and abilities by providing a clear illustration of where a client has strengths, as well as where to focus additional energy to generate improvement. Lastly, it is a self-assessment tool for individuals who wish to determine their own strengths and areas for improvement as they work towards self-sufficiency.
